Insulin resistance index from fasting glucose and insulin. Under 1.5 is clean; over 2.0 flags a workup.
HOMA-IR (Homeostatic Model Assessment for Insulin Resistance) is a simple calculation: (fasting glucose mg/dL × fasting insulin uIU/mL) / 405. Under 1.5 is optimal, 1.5-2.0 is watch-list, over 2.0 flags meaningful insulin resistance, and over 2.5 often signals metabolic syndrome in the making. HOMA-IR moves earlier than HbA1c - usually 2-5 years - which is why it's the best cheap early-warning system for type 2 diabetes risk. Requires an overnight fast before the draw. Run it via the Advanced panel alongside ApoB and a lipid panel.
